Inline edit can't save first row

When displaying a grid, if I click on the first displayed row with a view to edit inline, the 'save' and 'cancel' icons are not visible in the Actions column. Columns 2 onwards are ok. Is this a bug?

Make sure the first column of grid must have unique alphanumeric data (usually a PK).
This is required for proper display as well as update & delete operations of particular row.
